Steps to record an annual AMIT cost base adjustment

Greetings I have an ETF (STW) for which I receive an Annual Attribution Managed Investment Trust (AMIT) statement. This requires an adjustment to the cost base (either an increase or decrease). I have to go back six years and make adjustments for each year. Can anyone advise the steps to make these adjustments to the cost base please.
