Duplicate times in timesheets

Timesheet rows are duplicated (ie same date/employee/client/project/item/time) and therefore too much time is invoiced to client. The duplicates do not show on the weekly or daily timesheets, only when listed in the Timesheets. This is a serious problem - we have over-invoiced clients.
