Bankfeed setup without Yodlee

Hi, I wish to setup the bank feed for a Savings account without using Yodlee.
The last time we configured a bank feed, there was a form available to do this. I need this form again as I am not going to use Yodlee.
Please provide me with the form.
I asked for this in another feed more than a month ago without response, I hope you respond this new query.
